- Psychiatric Evaluation by a psychiatrist/addictionologist
- Neuropsychological testing
- Neurological evaluation
- Biopsychosocial evaluation
- Comprehensive medical record review and summarization
- Coordination of brain imaging services
- Team feedback session with client and family
- Coordination of companion/coaching services
- Extensive medical and lab work up
Objective: To provide a comprehensive multidisciplinary concierge medical and behavioral health assessment program. This assessment program addresses the whole persons health and wellness. This approach has been created to identify root causes related to medical, psychological, social, spiritual and behavioral aspects of our patient’s lives. The assessment will be utilized to inform the most effective recommendations and coordinated treatment plan including providers and overall ongoing management.
